Official Rules
 
  1. No purchase necessary. Void where prohibited. Contest sponsored by North Jersey Media Group Inc. (“NJMG”), publisher of The Record. Contest entry period runs July 13, 2015 - October 5, 2015 at 11:59 p.m. E.S.T. Contest is open to persons nominating nurses who work in Bergen, Passaic, Essex, Sussex, Morris and Hudson counties in NJ  and who are licensed in NJ and in good standing. Officers, directors and employees of NJMG, its parent, subsidiaries, affiliates, and agencies, any member of the panel of judges for this Contest, and immediate family members and/or persons living in the same household as any of the foregoing persons may not be nominators or nominees. Nominators and nominees must be 18 years of age or older.
  2. Submission form is only available online at http://www.northjersey.com/nurses. To enter, fully complete the entry form and nominate a nurse in any of the eight categories provided: team  (such as ER or ICU) , pediatric, cardiac, oncology, volunteerism, visiting nurses, education and nursing administration. Nomination must specify the category for which the nurse is being nominated. A space is provided to give an explanation of your nomination. Nominations must be 250 words or less.
  3. Submission forms must be submitted through the Contest website by October 5, 2015 at 11:59 p.m. E.S.T. to be eligible . An explanation for each nomination must be included. A nominator does not have to nominate someone for all eight categories. All submissions will be reviewed by a panel of judges consisting of at least two nurses and personnel from NJMG. Submissions will be judged based on the following judging criteria: a. Outstanding achievements in the field b. Impact on patient’s lives c. Dedication to the profession d. Connection to the community e. Clarity and composition of nomination explanation
  4. The nomination(s) must be original and must have been created by the nominator. Submissions that plagiarize the work of others will be disqualified. NJMG reserves the right to verify the originality of the nomination and any facet of a submission. Winner must be a licensed nurse in good standing and currently working in Bergen, Passaic, Essex, Sussex, Morris and Hudson counties in New Jersey. By submitting a nomination, the nominator agrees to be bound by these rules. The panel’s decisions on all matters relating to this Contest are binding and final.
  5. Eight (8) winners (one winner in each of the nomination categories) will be chosen from all eligible submissions received based on the judging criteria noted in Paragraph 3. Winners will be chosen on or before October 16, 2015. Winners will receive a $100 Visa gift card and be honored in The Record’s Tribute to Nurses special section. The winners and up to 5 guests will be invited to a celebratory breakfast hosted by The Record on Monday, December 7, 2015 – location and time to be determined.
  6. Prize is non-transferable. Cash redemption, exchange, or substitution is not allowed, except at the sole discretion of NJMG. NJMG reserves the right to substitute a prize of equal or greater value. Taxes and/or fees are the sole responsibility of the winners.
  7. NJMG and its parent, subsidiaries, affiliates, and the officers, directors, employees, owners and agents of each of the foregoing entities are not responsible for and assume no liability for (a) any incorrect or inaccurate entry information, or for any faulty, failed, garbled or jumbled electronic data transmissions; (b) any unauthorized access to, or theft, destruction or alteration of entries at any point in the operation of the Contest; (c) any technical malfunction, failure, error, omission, interruption, deletion, defect, delay in operation or communications line failure, regardless of cause, with regard to any equipment, systems, networks, lines, satellites, servers, computers or providers utilized in any aspect of the operation of the Contest; (d) inaccessibility or unavailability of the Internet or any combination thereof; (e) any injury or damage to nominator’s or to any other person’s computer which may be related to or resulting from any attempt to participate in the Contest; (f) other technical malfunctions or disturbances or other communications failures or circumstances affecting receipt of entries or ability to participate in the Contest. If, for any reason, NJMG determines, in its sole and final discretion, it does not wish to proceed with the Contest or that the Contest is not capable of proceeding as planned for any reason, including without limitation, infection of any computer system by computer virus, bugs, tampering, unauthorized intervention, fraud, technical failures, or any other causes which may corrupt or affect the administration, security, fairness, integrity or proper conduct of this Contest, then NJMG reserves the right, in its sole discretion to void the entries at issue, or to terminate, modify or suspend the Contest in whole or in part.
  8.  Nominators will be notified by telephone on or about October 16, 2015 if his/her nominee has been selected as a winner. It is the nominator’s responsibility to contact the winner and to have the winner contact NJMG. If a winner fails to contact NJMG within 96 hours of NJMG’s notification of the nominator,, NJMG may in its sole discretion award the prize to an alternate nominee whose qualifications meet the criteria. No guarantee of notification is made to nominees who do not win the Contest. The winners must supply NJMG with valid I.D.   and sign an Affidavit of Eligibility/Liability & Publicity Release (“Affidavit”) and any other documents requested by NJMG. If the winner fails to provide I.D., fails to complete and sign the Affidavit or if prize notification is not answered/confirmed within 7 days of notification, that entry will be void and an alternate winner will selected in accordance with the criteria set forth in Paragraph 3.
  9. NJMG reserves the right to terminate, modify, or otherwise change this Contest, at its sole discretion, should it determine that its fairness has been compromised. NJMG reserves the right, in its sole discretion, to determine whether the Contest has been compromised. NJMG reserves the right to disqualify any nominator or nominee who has been found to have tampered with the Contest, submitted multiple entries, or submitted entries using any automated means. In the event NJMG determines, in its sole discretion, that (a) the entry service has been misused; or (b) that technical difficulties or unforeseen events outside NJMG’s control compromise the integrity or viability of the Contest, NJMG reserves the right to void the entries at issue and/or terminate the Contest. NJMG reserves the right to confirm that nominees hold valid and current licenses and work in the applicable counties set forth in paragraph _1_ above.
  10. Entry constitutes agreement to hold NJMG and its parent, subsidiaries, and affiliates, and the officers, directors, employees, owners, and agents of each of the foregoing entities, harmless from and against any and all damages, liability, losses, and expenses arising out of, or claimed to be arising out of, this Contest, acceptance or use of the prize, and/or any use, in any manner, of any entry materials by NJMG and/or its licensees.
  11. Contest subject to all federal, state and local laws and regulations. Void where taxed, restricted, or prohibited by law.
  12. By entering this contest, all nominators give consent to NJMG’s publication of his/her name, likeness, photo (if applicable), hometown, nomination information, and prize allocation (if any) regardless of whether the nominator’s nominee is the winner of the Contest. All nominators agree to sign any documents, requested by NJMG including  affidavits and documents regarding publication of photos. To be notified of the prizewinners, send a self-addressed, stamped envelope to: The Record Marketing Department, Tribute to Nurses Winners, 1 Garret Mountain Plaza, P.O. Box 471, Woodland Park, NJ. Requests must be received by April 30, 2016. 
  13. Entry constitutes agreement to be bound by all of the foregoing rules and terms and by the decision of the judges, and also constitutes permission to use the nominator’s name and likeness for advertising and promotional purposes, without compensation to or additional approval from the entrant, except where prohibited by law.
